Documents You Need to Join the Ukrainian Navy
  1. Candidate Application.
  2. Handwritten Autobiography.
  3. Printed Autobiography.
  4. Copy of Military ID or Copy of Registration Certificate.
  5. Copy of Passport (if plastic, provide copies of both sides and the certificate) (in three copies).
  6. Copy of International Passport (if available).
  7. Copy of Extract from the Demographic Register (in three copies).
  8. Copy of Tax Identification Number (in three copies).
  9. Copy of Birth Certificate.
  10. Copy of Diploma and Transcript (if multiple diplomas, it's preferable to provide copies of all with their corresponding transcripts).
  11. Reference (from the place of work, village council, HOA, etc.).
  12. Copy of Work Record Book or OK-5 form from the pension fund.
  13. Certificate from a Psychiatrist and Narcologist (on one F 100-2/0 form) for military service under contract.
  14. MIA Certificate (Full, from the Action app).
  15. Blood Type.
  16. Medical Card or Medical Record.
  17. Consent to Process Personal Data.
  18. File Folder.
  19. Photo 9x12 cm (1 piece) (for Navy positions, photo in formal Navy uniform) matte finish.
  20. Photo 3x4 cm (6 pieces) matte finish.
  21. Copy of Marriage Certificate (or Divorce Certificate).
  22. Copy of Children’s Birth Certificates.
  23. Copy of Driver's License (if available).
  24. Copy of Military Service ID (if available).
  25. Recommendation Letter.
  26. Gynecologist Certificate, issued within 72 hours prior to departure (for women).
